SSS ID Card Requirements
Are you on your way to apply for a SSS ID card? This short guide will help you in the process of your application from ID form and requirements to the tips on where you can find SSS branch with ID capture machine. The SSS form used for ID application is E-6.
SSS ID Card Application Requirements and Form
With respect to SSS ID capture machines, there is one for you sure you can find at the SSS main office which you can find at SSS Building East Avenue, Diliman Quezon City. For telephone numbers before you apply, please call (632) 920-6401, 920-6446 while you can also try to email them at – member_relations@sss.gov.ph. Makati also had their own ID capture machine as well as Manila branch.
Requirements for Application
Primary documents –
- Passport
- Professional Regulation Commission (PRC) card
- Seaman’s book
If you don’t have any of these primary documents, you may use the following as substitute as follows:
- Driver’s license
- Valid National Bureau of Investigation (NBI) clearance
- School or company ID
- Postal ID
- Senior citizen card
- Major credit card
- Voter’s ID
- Savings account passbook
- Alien certificate of registration
- Government Service Insurance System (GSIS) member’s record
- Certification from the Office of Southern/Northern Cultural Communities or Office of Muslim Affairs
- Taxpayer Identification Number (TIN) card
SSS ID form E-6 may be downloaded at the SSS downloadable forms section on their website. On the other hand, there is an extra charge for lost ID, damaged ID or for removal of the date of birth as per owner request. The usual mailing of the SSS ID is withing 30 days. IF you don’t receive it withing that period, please contact or inquire at your nearest SSS branch.

Sir/Madam, I applied for my SSS ID on July 2011 at your Cubao Branch. I haven’t received my ID till now. I just want to know the status. Any idea kung gaano talaga katagal ang processing ng ID. Hoping for your soonest response. Thanks.

Please help me with my concern. I had my SSS Id application in Cebu with my previous employer. The problem is that SSS already mailed my ID to my previous employer but our company was dissolved already early this year. I am now in another company and they need my SSS ID as my requirement for regularization. I asked my brother to see somebody in SSS Cebu to claim for it but they said its in the post office. He went to see Cebu Post office but they said they returned it to SSS. I do not know what to do. please help me with this. thank you so much

paanu po ang gagawin namin kumuha kami ng new id ng mister o pero pagdating main office ng sss binigyan lang kami ng skedule ng pagbalik para daw sa picture captured ang problema 3 months pa….ang skedule namin…after that time syempre pareho kaming busy nawala na sa isip namin yong sss…so anung dapat gawin namin ang layo namin sa main office ng sss..sobrang pahirap po ang ginagawa nyo sa mga tao.
at sana po kung maari maglagay kayo ng branches na pwde sila mag isyu ng new id ng sss…kung lahat ng tao pupunta sa main office at pababalikin nyo lang aba di na po mura ang pamasahe yong iba galing pang province di ba kayo naawa.sana ibalik na lang ulit sa id card yong yellow kahit papanu yong release agad.kung kelan nag improve yong id syang bagal at pangit ng patakaran….gawaan nyo ng paraan yan sayang yong malaking pera ng sss pinapasok sa sss kung ganyan ang gawain naman …di kayo nakaktulong sa bayan

Question regarding the sss id.. What if i don’t know the maiden name of my mother, i checked already their marriage certificate naka indicate lng sa signature nia letter G. d ko po alam kng anu un, wala na kasi ako communication sa mother ko since bata po kaya d ko alam even my father don’t know her maiden name. i was so upset with SSS. I have already applied last May 2010 for SSS I.D but until now i haven’t received the released one. Already followed up at SSS main many times since last October, pero lahat na daw ng i.d applied from 2009-2010 has been already released. Where might my SSS be found? Super bagal ng sistema nila, they even told me to visit the nearest SSS branch to check. Hello, do they mean i have to re-apply again?…Hope our government will give time to check this kind of very odd system at SSS. This is time consuming on our part but we have no choice left kasi we need this I.D… To SSS kindly do your part naman po. thanks!

ang bagal ng proseso nyo kailangan kahit may isang buwan palang na may hjulog bigay nyo na dapat ung ID..

Please confirm — if my sister has retirement benefits to claim, does she neeed to apply for her ID card ? If the ID card is needed, can she get the ID card within 1 month from submission of application? She is a senior citizen and is very weak, the procedural requirements of SSS are very inconient.

Mam follow up ko lang po sana yong sss id ko kung kaylan po ma release. nag apply po ako sa branch po ninyo sa pasay sabi nila papadala nalang daw po nila sa address na pinil up ko hanggang ngaun bakit wala parin po mag sex month na po hind parin na rerelease ilang month po ba talaga bago matapus ang id sana po na uunawaan ninyo ako. kaylangan ko rin po kc sa pag pa process ng mga files ko po umaasa po ako sa inyong serbisyo.

ask ko lang po kung kaylan pwedeng mag pa i.d dahil naka pag change status na ako.kasi po may sss i.d ako ng dalaga ako.mga ilang months po ba bago makuha ang sss i.d.ask ko din po kung magkano ang penson ng naka 125 months.pero ang contribution ko po ay 520/per month.salamat po.

Naginquire ako nung December 2011 kc nagapply ako nung June 2011… and the reaction of the personel was like “Duh! 2010 pa nga lng po ang pinaprocess nmin as of now…”

june 2010 pa po ako nag-apply ng sss id sa antipolo branch.. march 2012 na po wala pa rin.. tumatawag po ako sa id center nyo, sinasagot ng operator pero pag nagpapaconnect na ko napuputol na.. sana masagot niyo po ang katanungan ko.. please reply.. thanks
